The Transition Nurse: A full time key worker charged with providing expert one-on-one care for new residents. In collaboration with the Care Manager, the Transition Nurse is responsible for ensuring that the needs and desires of the new resident and their family are clearly understood and documented to inform their individual care plan.
The Clinical Nurse Consultant : A full time expert nursing consultant who supports the registered nurses, their supervisor and the Village Care Manager. Areas of expertise include pain management, wound care and palliation.
The Clinical Nurse Educator : A full time role in every Village providing ongoing professional development and training for clinical staff.
The Care Manager: A full time role in every Village responsible for leading teams to provide quality resident care through training, education and management of staff, accreditation and continuous improvement.
